Your First 30 Days with PACE
Week 1: Getting Started
Your journey begins with comprehensive health assessments and introductions to your care team. This helps us understand your medical needs, daily routines, and personal goals.
Week 2: Your Personalized Care Plan
Our interdisciplinary team works together to create a care plan tailored to you. We review the plan with you and your caregiver and make sure it fits your lifestyle and preferences.
Week 3: Routine Care Begins
Your scheduled services begin, including medical appointments, therapy, and coordinated care at the center, at home, or with outside specialists. Transportation to and from appointments is provided.
Week 4: Check-Ins and Adjustments
We check in after your first month to see how everything is going and make any needed adjustments. Your care plan is flexible and designed to grow with you.
